Most commonly performed dental procedures are listed on the Fee Schedule. Only procedures performed at contracted offices are covered under this plan. Any procedure not listed on the plan fee schedule will be offered a 15-20% discount of the provider's usual fee. Implant restorations not listed are given a 10% discount.

WHO IS ELIGIBLE TO ENROLL?
Single individuals, domestic partners, married couples and their dependents are eligible. Dependents are eligible between 4 and 26 years of age (regardless of whether the dependent is attending school, living outside the parents' home, or married). Membership may be continued for a dependent over the age of 26, if the dependent is incapable of self-sustaining employment by reason of developmental or physical handicap. Parents or guardians who would like to enroll children or dependents on their own plan may do so.

WHAT IF I HAVE ANOTHER DENTAL PLAN?
Southwest Dental Plan may be used after you have reached your maximum coverage allowance with your current dental plan, or when any procedure is not covered by your dental insurance provider.

IS SPECIALTY CARE INCLUDED?
Yes. Southwest Dental Plan covers specialty care, such as Endodontics, Periodontics, Oral Surgery, TMJ, and Orthodontia. Fees are listed in the Specialty Care Fee Schedule.

WHEN DOES THE PLAN GO INTO EFFECT?
Membership in Southwest Dental Plan can become effective the same day the enrollment application and membership fee are received, although it normally takes 5 to 7 working days to process your application. You will receive your membership card shortly thereafter. If you have an emergency before receiving your membership card, call 480-745-1577 and we will verify eligibility.
